Rearranges the logic to properly initialize the manager's child id and renderer id maps after pre-initializing but before initializing children. Also, ensures that the focused item gets set before initializing children.
Since we use |focus_| in BrowserAccessibilityManager as a test whether |instance| is focused, we need to call SetFocus() before initializing children.

- Implement OpenFromURL. This allows you to right-click a link in a popup and
choose "open in new window".
- Refactor AddNewContents to remove duplicate code. Most codepaths now go

and cookies. The regression caused incorrect proxy information to be returned back to the plugin (java).
For the curious we were incorrectly assigning a function returning a bool to an std string which was eventually
used to return the proxy info back to the plugin.

There used to be a bug which caused us to count conflicts incorrectly. We
would count each conflict or conflict set once (by adding two to the count),
then decrement it by one. The problem is that the increment happened only
whent he conflict resolution function was called for the worker to which the
conflict belonged, while the decrement would be carried out by each worker.
This meant that the conflict never got very high.
Fixing issue 97832 brought this code back from the dead. The number of workers
involved in a sync cycle was greatly reduced. So the counts actually could
exceed 2 and the extra-special, tough conflict resolution code could get
triggered. This side-effect was completely unintentional.
We've decided that we would like to go back to the old, bug-induced behaviour.
This commit removes the buggy counting code and the special case, called only
on high count conflict processing functions. This should restore the old
behaviour that we've grown attached to.
This change also obsoletes issue 46621. The code to be tested by that unit
test has been deleted.

It doesn't recover from a stopped device yet. That would require completely reinitializing D3D.
Some cases also don't work yet. For example, if an async present fails, although it resets the device, it doesn't ask the renderer to produce a new frame. I'll hook that up to the completion callback or something.

UITest::CreateProxyLauncher() creates a proxy launcher that severes
the IPC connection to the browser process on AutomationProxy::Send().
If a bug makes the browser misbehave on startup, the IPC connection
is dropped immediately, which means that
ProxyLauncher::WaitForBrowserProcessToQuit() will always timeout.
Fix this by assuming that waiting for shutdown will always fail if
the IPC connection was prematurely dropped.
