This family of classes enable presenting media combined from multiple URLs
(e.g. pulling audio from one URL and video from another). In the future it
will also enable switching sources during playback, but this isn't implemented
yet.
player_{x11,wtl} have been updated to use these new classes, but
webmediaplayer_impl hasn't yet, because I don't want to expose my hacky
alternative scheme to the world.

PipelineError was a poor naming choice because most of the time variables of
that type held the value PIPELINE_OK meaning there was in fact no error.
Replaced the idiom of [0-ary callback + GetError()] with
[1-ary callback taking PipelineStatus argument] which makes the Pipeline API
cleaner and less error-prone. Before, consumers of the API had to make sure to
call GetError() at the top of each callback, or risk missing state transitions
in the pipeline. Now each callback gets an explicit parameter holding the
pipeline status at the moment the callback was invoked so failing to handle
error conditions should be more apparent in the code.

PipelineImpl no longer knows about DataSources, with DemuxerFactory taking a
DataSourceFactory when appropriate (i.e. for FFmpegDemuxerFactory).
This diff is relative to patchset 8 of issue 6480050, which must be submitted
first.

separate Filter management from MessageLoop management.
This sets the stage for filters to share threads in the
future which will reduce resource consumption when
multiple <video> tags are on the same page.

This enforces that the Singleton<T> pattern can only be used within classes which want singleton-ness.
As part of this CL I have also fixed up files which got missed in my previous CLs to use a GetInstance() method and use Singleton<T> from the source file.
There are a small number of places where I have also switched to LazyInstance as that was more appropriate for types used in a single source file.

MediaFilterCollection is the only code that uses FilterType now so
I'm moving the enum into that class and removing all references from
the MediaFilter & derived classes.

FilterFactory, IsMediaFormatSupported and CreateFactory are the source
of evil. They also have have a gang of template functions. This patch
terminate them all and make the world a better place.

which VDC to be used should be determined by something outside instead of inside video decoder.
1. whether hardware composition is used( probably always true).
2. whether hardware composition is disabled.(if not, why we keep the skbitmap path).
3. whether a opengl or a opengles VDC is needed, probably only apply to player_x11.
4. the video decoder context are sharing a lot of states with "painter/compositor" (e.g. graphic context). and if we create VDC inside decoder, and there is no current code path to inject this to Renderer, there are also complication of the factories and creating orders. even we add that path, it maybe too late for renderer.
while we need to refactor ipc_video_decoder later.

Some video streams are badly muxed and have video packets
with timestamps greater than the duration. This causes the
logic in the video renderer to sleep forever and never read
the end-of-stream packet and thus the stream is never ended.
This change adjusted the condition on when we should sleep
and handle timestamp greater than duration as a special case.
