DnsCertProvenanceChecker is introduced to abstract away the action of
uploading reports. It's implemented by ChromeDnsCertProvenanceChecker
which lives in a scoped_ptr off of a URLRequestContext.
It's only active on the main context (i.e. not in incognito mode). This
might change in the future, but it's a nice, conservative choice for the
moment.
The DnsCertProvenanceChecker is plumbed all the way to SSLClientSocket
(via HttpCache) where it replaces the DnsRRResolver. Above
SSLClientSocket, it's plumbed in addition to the DnsRRResolver because
the later will end up going into SSLHostInfo in the future.
At the moment, ChromeDnsCertProvenanceCheckerContext is just a skeleton
to avoid putting too much real code in a plumbing job.

base::debug namespace.
This splits apart debug_util into base/debugger and base/stack_trace
There are still two functions in debug_util that I'm not sure what to do with.
Since this uses the base::debug namespace, I removed the functions in
debugger.h from the static class and just made them free functions in the
namespace.

Also converts COOKIE_DLOG (since VLOG can be toggled on a granular basis), removes some unneeded "endl"s and {}s, aligns "<<"s per style guide, and changes KeygenHandler::GenKeyAndSignChallenge() to avoid using "goto".

This is a revert of r62107 which is a revert of r62105 which is a revert of r60753 which is a revert of r60739 which is a revert of r60025 which is a revert of r59972 which is a revert of r59570 which is a revert of r59511 which is a revert of r59299 which is a revert of r59289.
The most recent reverts have all been due to ChromiumOS leaks which I believe to be fixed after fixing bug 58572, since I tested the combined fix in r62105 which was green on all ChromiumOS valgrind bots.

Previously the default registry factory would crash if it saw a Negotiate header when trying to do a CNAME lookup because of a NULL resolver. Since then I've changed HttpAuthHandlerNegotiate to skip the CNAME lookup step when the resolver_ is NULL.
So, one option is to make CreateDefault accept an optional HostResolver argument. My concern is that since passing in NULL is the easier choice, people will do it and encounter user problems when going through a Negotiate proxy. Hence, I force the correct behavior.

Since we can have multiple profiles, this causes the initialization (and uninitialization) to happen multiple times, which causes problems for OCSP since we use statics. The solution is to identify the "main" Profile. We create said Profile in BrowserMain. I add an "is_main_" variable to URLRequestContextGetter and URLRequestContext, so that only the "main" URLRequestContext will initialize OCSP.
The only change from r59972 is the location that I set_is_main(). I set it when we set the default request context. This is because, by the time I had called set_is_main() in BrowserMain, the ChromeURLRequestContextGetter was already created (it gets created during the CreateProfile() function). So I was too late.
