This also makes the default be threadsafe. The old factory wasn't threadsafe even to the extent claimed in the header which was causing hangs in plugins

Previously this was only getting sent when the page had focus, which is pretty
useless. This patch also clarifies some API documentation.

I changed these to take InstanceHandles but this doesn't actually work. If you
use them the recommended way, the module->instance mapping isn't set up by the
time the constructors are called, so doing AddPerInstanceObject fails.
Since these objects are all called on the main thread, there is no race
condition with pp::Instance destruction and the implementation using a pointer
is safe.

This will allow us to be more flexible about adding data to view changed updates in the future. For now, I've incorporated fullscreen and tab foreground state into the view state.

wheel events to remove the redundant "Mouse" and "Wheel" in the getters.
Rename PP_INPUTEVENT_TYPE_MOUSEWHEEL to .._WHEEL so that it doesn't look
like a mouse event (it's its own type).
Add documentation to the C++ wrapper from the C wrappers. Do some minor
documentation updates.
Update IDL for some of the recent changes we've made. Update the way points
are passed for resource creation since IDL wanted to do it by pointer.

This converts the input event from a C struct to a resource to give us more
ability to change over time. This patch includes a proxy and a C++ wrapper for
this resource.
You now have to register for classes of input events. No events are sent by
default. This also allows us to specify whether the events support bubbling
or not, which allows us to better-optimize IPC.

this adds discussion of the fast shutdown mode where the cleanup functions won't get called.
I also copied the instance documentation to the C++ class since apparently we want the C++ layer to be documented in isolation.

Create 0.5 versions of the instance interfaces that are the default only if PPAPI_INSTANCE_REMOVE_SCRIPTING is defined. This is so [hopefully] plugins can opt in to the new version and migrate to InstancePrivate/VarPrivate more easily.

interfaces will allow JavaScript to send data asynchronously to a module instance, and the module instance to asynchronously send data to a JavaScript message handler.
Note, I did something differently from other per-instance interfaces. While the C interface has 'PPB_Messaging' and 'PPP_Messaging' separate from the Instance interfaces, I stuck the per-instance messaging in to pp::Instance. It seems more intuitive to me, and doesn't have the drawbacks of having too many functions in the C layer instance interfaces. Happy to back off of that position, but it's worth a shot.
Overview:
From JavaScript, you can invoke 'postMessage' on the embedded module. That results in a call to 'PPP_Messaging::HandleMessage'.
From Native Code, you can invoke 'PPB_Messaging::PostMessage', which results
in a call to an 'onmessage' function on the DOM element for the module
instance in the JavaScript code (if one has been registered).
Please see the included example or the examples in the comments of
PPB_Messaging and PPP_Messaging.
Restrictions:
- This implementation is synchronous. A later CL will make it asynchronous.
- This implementation supports only intrinsic values and strings (all types that PP_Var supports except for objects). Object & array support will come later.
- This implementation only allows for 1 channel per instance. You can not expose other 'channels' or 'ports'. Future CLs will add support for MessagePorts.

- Fixed a bug where GLES2 decoder set texture state without updating TextureInfo.
- Fixed Pepper3D demos and added awesome 3D CSS programmer art.
- Removed ggl::GetCurrentContext to prevent further abuse. Fixed said abuse :)
- GGL exposes GLES2Implementation to allow issue of GL calls on a particular context without making that context globally current.
- Removed redundant "this context" argument from GGL SwapBuffers completion callback.
- Temporarily removed context lost notification. I need to figure out the best semantics.
