The IRB might be 96 bytes if the extended-I/O-measurement facility is
used. This feature is currently not used by Linux, but struct irb
already has the emw defined. So let's make the irb in lowcore match the
size of the internal data structure to be future proof.
We also have to add a pad, to correctly align the paste.
The bigger irb field also circumvents a bug in some QEMU versions that
always write the emw field on test subchannel and therefore destroy the
paste definitions of this CPU. Running under these QEMU version broke
some timing functions in the VDSO and all users of these functions,
e.g. some JREs.

Commit b4cbb197c7e7 ("vm: add vm_iomap_memory() helper function") added
a helper function wrapper around io_remap_pfn_range(), and every other
architecture defined it in <asm/pgtable.h>.
The s390 choice of <asm/io.h> may make sense, but is not very convenient
for this case, and gratuitous differences like that cause unexpected errors like this:

flush_signal_handlers() needs to know whether sigaction::sa_restorer
is defined, not whether SA_RESTORER is defined. Define the
__ARCH_HAS_SA_RESTORER macro to indicate this.

Our flush_tlb_kernel_range() implementation calls __tlb_flush_mm() with
&init_mm as argument. __tlb_flush_mm() however will only flush tlbs
for the passed in mm if its mm_cpumask is not empty.
For the init_mm however its mm_cpumask has never any bits set. Which in
turn means that our flush_tlb_kernel_range() implementation doesn't
work at all.
This can be easily verified with a vmalloc/vfree loop which allocates
a page, writes to it and then frees the page again. A crash will follow
almost instantly.
To fix this remove the cpumask_empty() check in __tlb_flush_mm() since
there shouldn't be too many mms with a zero mm_cpumask, besides the
init_mm of course.

Converting a 64 Bit TOD format value to nanoseconds means that the value
must be divided by 4.096. In order to achieve that we multiply with 125
and divide by 512.
When used within sched_clock() this triggers an overflow after appr.
417 days. Resulting in a sched_clock() return value that is much smaller
than previously and therefore may cause all sort of weird things in
subsystems that rely on a monotonic sched_clock() behaviour.
To fix this implement a tod_to_ns() helper function which converts TOD
values without overflow and call this function from both places that
open coded the conversion: sched_clock() and kvm_s390_handle_wait().

If user space is running in primary mode it can switch to secondary
or access register mode, this is used e.g. in the clock_gettime code
of the vdso. If a signal is delivered to the user space process while
it has been running in access register mode the signal handler is
executed in access register mode as well which will result in a crash
most of the time.
Set the address space control bits in the PSW to the default for the
execution of the signal handler and make sure that the previous
address space control is restored on signal return. Take care
that user space can not switch to the kernel address space by
modifying the registers in the signal frame.

The downgrade of the 4 level page table created by init_new_context is
currently done only in start_thread31. If a 31 bit process forks the
new mm uses a 4 level page table, including the task size of 2<<42
that goes along with it. This is incorrect as now a 31 bit process
can map memory beyond 2GB. Define arch_dup_mmap to do the downgrade
after fork.

Git commit 36409f6353fc2d7b6516e631415f938eadd92ffa "use generic RCU
page-table freeing code" introduced a tlb flushing bug. Partially revert
the above git commit and go back to s390 specific page table flush code.
For s390 the TLB can contain three types of entries, "normal" TLB
page-table entries, TLB combined region-and-segment-table (CRST) entries
and real-space entries. Linux does not use real-space entries which
leaves normal TLB entries and CRST entries. The CRST entries are
intermediate steps in the page-table translation called translation paths.
For example a 4K page access in a three-level page table setup will
create two CRST TLB entries and one page-table TLB entry. The advantage
of that approach is that a page access next to the previous one can reuse
the CRST entries and needs just a single read from memory to create the
page-table TLB entry. The disadvantage is that the TLB flushing rules are
more complicated, before any page-table may be freed the TLB needs to be
flushed.
In short: the generic RCU page-table freeing code is incorrect for the
CRST entries, in particular the check for mm_users < 2 is troublesome.
This is applicable to 3.0+ kernels.

Commit 2a95ea6c0d129b4 ("procfs: do not overflow get_{idle,iowait}_time
for nohz") did not take into account that one some architectures jiffies
and cputime use different units.
This causes get_idle_time() to return numbers in the wrong units, making
the idle time fields in /proc/stat wrong.
Instead of converting the usec value returned by
get_cpu_{idle,iowait}_time_us to units of jiffies, use the new function
usecs_to_cputime64 to convert it to the correct unit of cputime64_t.

This patch makes sure we don't underindicate _PAGE_CHANGED in case
we have a race between an operation that changes the page and this
code path that hits us between page_get_storage_key and
page_set_storage_key. Note that we still have a potential
underindication on _PAGE_REFERENCED in the unlikely event that
the page was changed but not referenced _and_ someone references
the page in the race window. That's not considered to be a problem.

CPUSTAT_RUNNING was implemented signifying that a vcpu is not stopped.
This is not, however, what the architecture says: RUNNING should be
set when the host is acting on the behalf of the guest operating
system.
CPUSTAT_RUNNING has been changed to be set in kvm_arch_vcpu_load()
and to be unset in kvm_arch_vcpu_put().
For signifying stopped state of a vcpu, a host-controlled bit has
been used and is set/unset basically on the reverse as the old
CPUSTAT_RUNNING bit (including pushing it down into stop handling
proper in handle_stop()).

In ESA mode STCKF is not defined even if the facility bit is enabled.
To prevent an illegal operation we must also check if we run a 64 bit kernel.
To make the check perform well add the STCKF bit to the machine flags.

The pgste_update_all / pgste_update_young and pgste_set_pte need to
check if the pte entry contains a valid page address before the storage
key can be accessed. In addition pgste_set_pte needs to set the access
key and fetch protection bit of the new pte entry, not the old entry.

We use both the external call and emergency call IPIs to signal remote
cpus. Therefore it makes sense to account them differently withing
/proc/irqstats so we actually know what happened.

gmap_fault needs to walk the guest page table. However, parts of
that may change if some other thread does munmap. In that case
gmap_unmap_notifier will also unmap the corresponding parts from
the guest page table. We need to take mmap_sem in order to serialize
these operations.
do_exception now calls __gmap_fault with mmap_sem held which does
not get exported to modules. The exported function, which is called
from KVM, now takes mmap_sem.

The user space program can change its addressing mode between the
24-bit, 31-bit and the 64-bit mode if the kernel is 64 bit. Currently
the kernel always forces the standard amode on signal delivery and
signal return and on ptrace: 64-bit for a 64-bit process, 31-bit for
a compat process and 31-bit kernels. Change the signal and ptrace code
to allow the full range of addressing modes. Signal handlers are
run in the standard addressing mode for the process.
One caveat is that even an 31-bit compat process can switch to the
64-bit mode. The next signal will switch back into the 31-bit mode
and there is no room in the 31-bit compat signal frame to store the
information that the program came from the 64-bit mode.

Split out addressing mode bits from PSW_BASE_BITS, rename PSW_BASE_BITS
to PSW_MASK_BASE, get rid of psw_user32_bits, remove unused function
enabled_wait(), introduce PSW_MASK_USER, and drop PSW_MASK_MERGE macros.
Change psw_kernel_bits / psw_user_bits to contain only the bits that
are always set in the respective mode.

Add an explicit TIF_SYSCALL bit that indicates if a task is inside
a system call. The svc_code in the pt_regs structure is now only
valid if TIF_SYSCALL is set. With this definition TIF_RESTART_SVC
can be replaced with TIF_SYSCALL. Overall do_signal is a bit more
readable and it saves a few lines of code.

An instruction with an address right below the adress limit for the
current addressing mode will wrap. The instruction restart logic in
the protection fault handler and the signal code need to follow the
wrapping rules to find the correct instruction address.

For a ERESTARTNOHAND/ERESTARTSYS/ERESTARTNOINTR restarting system call
do_signal will prepare the restart of the system call with a rewind of
the PSW before calling get_signal_to_deliver (where the debugger might
take control). For A ERESTART_RESTARTBLOCK restarting system call
do_signal will set -EINTR as return code.
There are two issues with this approach:
1) strace never sees ERESTARTNOHAND, ERESTARTSYS, ERESTARTNOINTR or
ERESTART_RESTARTBLOCK as the rewinding already took place or the
return code has been changed to -EINTR
2) if get_signal_to_deliver does not return with a signal to deliver
the restart via the repeat of the svc instruction is left in place.
This opens a race if another signal is made pending before the
system call instruction can be reexecuted. The original system call
will be restarted even if the second signal would have ended the
system call with -EINTR.
These two issues can be solved by dropping the early rewind of the
system call before get_signal_to_deliver has been called and by using
the TIF_RESTART_SVC magic to do the restart if no signal has to be
delivered. The only situation where the system call restart via the
repeat of the svc instruction is appropriate is when a SA_RESTART
signal is delivered to user space.
Unfortunately this breaks inferior calls by the debugger again. The
system call number and the length of the system call instruction is
lost over the inferior call and user space will see ERESTARTNOHAND/
ERESTARTSYS/ERESTARTNOINTR/ERESTART_RESTARTBLOCK. To correct this a
new ptrace interface is added to save/restore the system call number
and system call instruction length.

This patch implements the crash_map_pages() function for s390.
KEXEC_CRASH_MEM_ALIGN is set to HPAGE_SIZE, in order to support
kernel mappings that use large pages. We also use HPAGE_SIZE alignment
for CONFIG_HUGETLB_PAGE=n in order to have the same 1 MiB alignment on
all s390 systems.

This patch defines for s390 an ABI defined pointer to the vmcoreinfo note at
a well known address. With this patch tools are able to find this information
in dumps created by stand-alone or hypervisor dump tools.

PSW restart can be triggered on offline CPUs. If this happens, currently
the PSW restart code fails, because functions like smp_processor_id()
do not work on offline CPUs. This patch fixes this as follows:
If PSW restart is triggered on an offline CPU, the PSW restart (sigp restart)
is done a second time on another CPU that is online and the old CPU is
stopped afterwards.

Current IRQ statistics support does not show detail counts for I/O
interrupts which are processed internally only. The result is a
summation count which is way off such as this one:

Implement sigp external call, which might be required for guests that
issue an external call instead of an emergency signal for IPI.
This fixes an issue with "KVM: unknown SIGP: 0x02" when booting
such an SMP guest.

This was found by inspection while tracking a similar
bug in compat_statfs64, that has been fixed in mainline
since decemeber.
- This fixes a bug where not all of the f_spare fields
were cleared on mips and s390.
- Add the f_flags field to struct compat_statfs
- Copy f_flags to userspace in case someone cares.
- Use __clear_user to copy the f_spare field to userspace
to ensure that all of the elements of f_spare are cleared.
On some architectures f_spare is has 5 ints and on some
architectures f_spare only has 4 ints. Which makes
the previous technique of clearing each int individually
broken.
I don't expect anyone actually uses the old statfs system
call anymore but if they do let them benefit from having
the compat and the native version working the same.

598841ca9919d008b520114d8a4378c4ce4e40a1 ([S390] use gmap address
spaces for kvm guest images) changed kvm to use a separate address
space for kvm guests. This address space was switched in __vcpu_run
In some cases (preemption, page fault) there is the possibility that
this address space switch is lost.
The typical symptom was a huge amount of validity intercepts or
random guest addressing exceptions.
Fix this by doing the switch in sie_loop and sie_exit and saving the
address space in the gmap structure itself. Also use the preempt
notifier.
