Fixed by taking the mutex in blktrans_open and blktrans_release.
Note that this locking is already suggested in
include/linux/mtd/blktrans.h:

Running mtd-utils/tests/ubi-tests/io_basic.c could cause
soft lockup or watchdog reset. It is because *updatevol*
will perform ubi_check_volume() after updating finish
and this function will full scan the updated lebs if the
volume is initialized as STATIC_VOLUME.
This patch adds *cond_resched()* in the loop of lebs scan
to avoid soft lockup.

In some of the 'out_not_moved' error paths, lnum may be used
uninitialized. Don't ignore the warning; let's fix it.
This uninitialized variable doesn't have much visible effect in the end,
since we just schedule the PEB for erasure, and its LEB number doesn't
really matter (it just gets printed in debug messages). But let's get it
straight anyway.

The logic of vfree()'ing vol->upd_buf is tied to vol->updating.
In ubi_start_update() vol->updating is set long before vmalloc()'ing
vol->upd_buf. If we encounter a write failure in ubi_start_update()
before vmalloc() the UBI device release function will try to vfree()
vol->upd_buf because vol->updating is set.
Fix this by allocating vol->upd_buf directly after setting vol->updating.

It seems that the corrupted partition header on my mtd device triggers
a bug in the ftl. In function build_maps() it will allocate the buffers
needed by the mtd partition, but if something goes wrong such as kmalloc
failure, mtd read error or invalid partition header parameter, it will
free all allocated buffers and then return non-zero. In my case, it
seems that partition header parameter 'NumTransferUnits' is invalid.
And the ftl_freepart() is a function which free all the partition
buffers allocated by build_maps(). Given the build_maps() is a self
cleaning function, so there is no need to invoke this function even
if build_maps() return with error. Otherwise it will causes the
buffers to be freed twice and then weird things would happen.

[1] The gpmi uses the nand_command_lp to issue the commands to NAND chips.
The gpmi issues a DMA operation with gpmi_cmd_ctrl when it handles
a NAND_CMD_NONE control command. So when we read a page(NAND_CMD_READ0)
from the NAND, we may send two DMA operations back-to-back.
If we do not serialize the two DMA operations, we will meet a bug when
1.1) we enable CONFIG_DMA_API_DEBUG, CONFIG_DMADEVICES_DEBUG,
and CONFIG_DEBUG_SG.
1.2) Use the following commands in an UART console and a SSH console:

1.3) Assume the two DMA operations is X (first) and Y (second).
The root cause of the bug:
Assume process P issues DMA X, and sleep on the completion
@this->dma_done. X's tasklet callback is dma_irq_callback. It firstly
wake up the process sleeping on the completion @this->dma_done,
and then trid to unmap the scatterlist S. The waked process P will
issue Y in another ARM core. Y initializes S->sg_magic to zero
with sg_init_one(), while dma_irq_callback is unmapping S at the same
time.

This patch fixes two memory errors:
1. During a probe failure (in mtd_device_parse_register?) the command
buffer would not be freed.
2. The command buffer's size is determined based on the 'fast_read'
boolean, but the assignment of fast_read is made after this
allocation. Thus, the buffer may be allocated "too small".
To fix the first, just switch to the devres version of kzalloc.
To fix the second, increase MAX_CMD_SIZE unconditionally. It's not worth
saving a byte to fiddle around with the conditions here.
This problem was reported by Yuhang Wang a while back.

Some bright specification writers decided to write this in the ONFI spec
(from ONFI 3.0, Section 3.1):
"The number of blocks and number of pages per block is not required to
be a power of two. In the case where one of these values is not a
power of two, the corresponding address shall be rounded to an
integral number of bits such that it addresses a range up to the
subsequent power of two value. The host shall not access upper
addresses in a range that is shown as not supported."
This breaks every assumption MTD makes about NAND block/chip-size
dimensions -- they *must* be a power of two!
And of course, an enterprising manufacturer has made use of this lovely
freedom. Exhibit A: Micron MT29F32G08CBADAWP
"- Plane size: 2 planes x 1064 blocks per plane
- Device size: 32Gb: 2128 blockss [sic]"
This quickly hits a BUG() in nand_base.c, since the extra dimensions
overflow so we think it's a second chip (on my single-chip setup):

It could happen (1 out of 100 times) that NAND did not start up
correctly after warm rebooting, so the kernel could not find the UBI or
DMA timed out due to a stalled BCH. When resetting BCH together with
GPMI, the issue could not be observed anymore (after 10000+ reboots). We
probably need the consistent state already before sending any command to
NAND, even when no ECC is needed. I chose to keep the extra reset for
BCH when changing the flash layout to be on the safe side.

The pointer returned by kzalloc should be tested for NULL
to avoid potential NULL pointer dereference later. Incorrect
pointer was being tested for NULL. Bug introduced by commit fbcf62a3
(mtd: physmap_of: move parse_obsolete_partitions to become separate
parser).
This patch fixes this bug.

The NAND_CHIPOPTIONS_MSK has limited utility and is causing real bugs. It
silently masks off at least one flag that might be set by the driver
(NAND_NO_SUBPAGE_WRITE). This breaks the GPMI NAND driver and possibly
others.
Really, as long as driver writers exercise a small amount of care with
NAND_* options, this mask is not necessary at all; it was only here to
prevent certain options from accidentally being set by the driver. But the
original thought turns out to be a bad idea occasionally. Thus, kill it.
Note, this patch fixes some major gpmi-nand breakage.

Unloading the omap2 nand driver missed to release the memory region which will
result in not being able to request it again if one want to load the driver
later on.

Do not kfree() the mtd_info; it is handled in the mtd subsystem and
already freed by nand_release(). Instead kfree() the struct

We may cause a memory leak when the @types has more then one parser.
Take the `default_mtd_part_types` for example. The default_mtd_part_types has
two parsers now: `cmdlinepart` and `ofpart`.
Assume the following case:
The kernel command line sets the partitions like:
#gpmi-nand:20m(boot),20m(kernel),1g(rootfs),-(user)
But the devicetree file(such as arch/arm/boot/dts/imx28-evk.dts) also sets
the same partitions as the kernel command line does.
In the current code, the partitions parsed out by the `ofpart` will
overwrite the @pparts which has already set by the `cmdlinepart` parser,
and the the partitions parsed out by the `cmdlinepart` is missed.
A memory leak occurs.
So we should break the code as soon as we parse out the partitions,
In actually, this patch makes a priority order between the parsers.
If one parser has already parsed out the partitions successfully,
it's no need to use another parser anymore.

UBI was mistakingly using 'kfree()' instead of 'kmem_cache_free()' when
freeing "attach eraseblock" structures in vtbl.c. Thankfully, this happened
only when we were doing auto-format, so many systems were unaffected. However,
there are still many users affected.
It is strange, but the system did not crash and nothing bad happened when
the SLUB memory allocator was used. However, in case of SLOB we observed an
crash right away.
This problem was introduced in 2.6.39 by commit
"6c1e875 UBI: add slab cache for ubi_scan_leb objects"
A note for stable trees:
Because variable were renamed, this won't cleanly apply to older kernels.
Changing names like this should help:
1. ai -> si
2. aeb_slab_cache -> seb_slab_cache
3. new_aeb -> new_seb

We don't need to open code the divide function, just use div_u64 that
already exists and do the same job. While this is a straightforward
clean up, there is more to that, the real motivation for this.
While building on a cross compiling environment in armel, using gcc
4.6.3 (Ubuntu/Linaro 4.6.3-1ubuntu5), I was getting the following build
error:
ERROR: "__aeabi_uldivmod" [drivers/mtd/nand/nandsim.ko] undefined!
After investigating with objdump and hand built assembly version
generated with the compiler, I narrowed __aeabi_uldivmod as being
generated from the divide function. When nandsim.c is built with
-fno-inline-functions-called-once, that happens when
CONFIG_DEBUG_SECTION_MISMATCH is enabled, the do_div optimization in
arch/arm/include/asm/div64.h doesn't work as expected with the open
coded divide function: even if the do_div we are using doesn't have a
constant divisor, the compiler still includes the else parts of the
optimized do_div macro, and translates the divisions there to use
__aeabi_uldivmod, instead of only calling __do_div_asm -> __do_div64 and
optimizing/removing everything else out.
So to reproduce, gcc 4.6 plus CONFIG_DEBUG_SECTION_MISMATCH=y and
CONFIG_MTD_NAND_NANDSIM=m should do it, building on armel.
After this change, the compiler does the intended thing even with
-fno-inline-functions-called-once, and optimizes out as expected the
constant handling in the optimized do_div on arm. As this also avoids a
build issue, I'm marking for Stable, as I think is applicable for this
case.

UBI error: io_init: bad write buffer size 0 for 1 min. I/O unit
We need to initialize writebufsize; I think "page_size" is the correct

The writebufsize concept was introduce by commit
"0e4ca7e mtd: add writebufsize field to mtd_info struct" and it represents
the maximum amount of data the device writes to the media at a time. This is
an important parameter for UBIFS which is used during recovery and which
basically defines how big a corruption caused by a power cut can be.
Set writebufsize to 4 because this drivers writes at max 4 bytes at a time.

However, we forgot to set this parameter for block2mtd. Set it to PAGE_SIZE
because this is actually the amount of data we write at a time.

In commit "c797533 mtd: abstract last MTD partition parser argument" the
third argument of "mtd_device_parse_register()" changed from start address
of the MTD device to a pointer to a struct.
The "ixp4xx_flash_probe()" function was not converted properly, causing
an oops during boot.
This patch fixes the problem by filling the needed information into a
"struct mtd_part_parser_data" and passing it to
"mtd_device_parse_register()".

The 'find_wl_entry()' function expects the maximum difference as the second
argument, not the maximum absolute value. So the "unknown" eraseblock picking

Two bad things can happen in ubi_scan():
1. If kmem_cache_create() fails we jump to out_si and call
ubi_scan_destroy_si() which calls kmem_cache_destroy().
But si->scan_leb_slab is NULL.
2. If process_eb() fails we jump to out_vidh, call
kmem_cache_destroy() and ubi_scan_destroy_si() which calls
again kmem_cache_destroy().

The reason is that it breaks 16 bits NAND flash as it was reported by
Nikolaus Voss and confirmed by Eric Bénard.

In MX28, if we do not reset the BCH module. The BCH module may
becomes unstable when the board reboots for several thousands times.
This bug has been catched in customer's production.
The patch adds some comments (some from Wolfram Sang), and fixes it now.
Also change gpmi_reset_block() to static.

Patch ab50ff684707031ed4bad2fdd313208ae392e5bb broke UBI debugging messages:
before that commit when UBI debugging was enabled, users saw few useful
debugging messages after attaching an MTD device. However, that patch turned
'dbg_msg()' into 'pr_debug()', so to enable the debugging messages users have
to enable them first via /sys/kernel/debug/dynamic_debug/control, which is
very impractical.
This commit makes 'dbg_msg()' to use 'printk()' instead of 'pr_debug()', just
as it was before the breakage.

Currently it's possible to create a volume without a name. E.g:
ubimkvol -n 32 -s 2MiB -t static /dev/ubi0 -N ""
After that vtbl_check() will always fail because it does not permit
empty strings.

When we fail to erase a PEB, we free the corresponding erase entry object,
but then re-schedule this object if the error code was something like -EAGAIN.
Obviously, it is a bug to use the object after we have freed it.

Some error paths in mtd_blkdevs were fixed in the following commit:

mtd: mtd_blkdevs: fix error path in blktrans_open
But on these error paths, the block device's `dev->open' count is
already incremented before we check for errors. This meant that, while
the error path was handled correctly on the first time through
blktrans_open(), the device is erroneously considered already open on
the second time through.
This problem can be seen, for instance, when a UBI volume is
simultaneously mounted as a UBIFS partition and read through its
corresponding gluebi mtdblockX device. This results in blktrans_open()
passing its error checks (with `dev->open > 0') without actually having
a handle on the device. Here's a summarized log of the actions and
results with nandsim:
