commit 5fb2c782f451a4fb9c19c076e2c442839faf0f76 upstream.
This device automatically switches itself to another mode (0x1405)
unless the specific access pattern of Windows is followed in its
initial mode. That makes a dirty unmount of the internal storage
devices inevitable if they are mounted. So the card reader of
such a device should be ignored, lest an unclean removal become
inevitable.
This replaces an earlier patch that ignored all LUNs of this device.
That patch was overly broad.

commit 93c9bf4d1838d5851a18ca398b0ad66397f05056 upstream.
Sometimes mass-storage devices using the Bulk-only transport will
mistakenly skip the data phase of a command. Rather than sending the
data expected by the host or sending a zero-length packet, they go
directly to the status phase and send the CSW.
This causes problems for usb-storage, for obvious reasons. The driver
will interpret the CSW as a short data transfer and will wait to
receive a CSW. The device won't have anything left to send, so the
command eventually times out.
The SCSI layer doesn't retry commands after they time out (this is a
relatively recent change). Therefore we should do our best to detect
a skipped data phase and handle it promptly.
This patch adds code to do that. If usb-storage receives a short
13-byte data transfer from the device, and if the first four bytes of
the data match the CSW signature, the driver will set the residue to
the full transfer length and interpret the data as a CSW.
This fixes Bugzilla #86611.

commit c80b4495c61636edc58fe1ce300f09f24db28e10 upstream.
This patch adds quirks for Entrega Technologies (later Xircom PortGear) USB-
SCSI converters. They use Shuttle Technology EUSB-01/EUSB-S1 chips. The
US_FL_SCM_MULT_TARG quirk is needed to allow multiple devices on the SCSI
chain to be accessed. Without it only the (single) device with SCSI ID 0
can be used.
The standalone converter sold by Entrega had model number U1-SC25. Xircom
acquired Entrega and re-branded the product line PortGear. The PortGear USB
to SCSI Converter (model PGSCSI) is internally identical to the Entrega
product, but later models may use a different USB ID. The Entrega-branded
units have USB ID 1645:0007, as does my Xircom PGSCSI, but the Windows and
Macintosh drivers also support 085A:0028.
Entrega also sold the "Mac USB Dock", which provides two USB ports, a Mac
(8-pin mini-DIN) serial port and a SCSI port. It appears to the computer as
a four-port hub, USB-serial, and USB-SCSI converters. The USB-SCSI part may
have initially used the same ID as the standalone U1-SC25 (1645:0007), but
later production used 085A:0026.
My Xircom PortGear PGSCSI has bcdDevice=0x0100. Units with bcdDevice=0x0133
probably also exist.
This patch adds quirks for 1645:0007, 085A:0026 and 085A:0028. The Windows
driver INF file also mentions 085A:0032 "PortStation SCSI Module", but I
couldn't find any mention of that actually existing in the wild; perhaps it
was cancelled before release?

commit b6a3ed677991558ce09046397a7c4d70530d15b3 upstream.
Hi,
The Ariston Technologies iConnect 025 and iConnect 050 (also known as e.g.
iSCSI-50) are SCSI-USB converters which use Shuttle Technology/SCM
Microsystems chips. Only the connectors differ; both have the same USB ID.
The US_FL_SCM_MULT_TARG quirk is required to use SCSI devices with ID other
than 0.
I don't have one of these, but based on the other entries for Shuttle/
SCM-based converters this patch is very likely correct. I used 0x0000 and
0x9999 for bcdDeviceMin and bcdDeviceMax because I'm not sure which
bcdDevice value the products use.

commit c66f1c62e85927357e7b3f4c701614dcb5c498a2 upstream.
The Iomega Jaz USB Adapter is a SCSI-USB converter cable. The hardware
seems to be identical to e.g. the Microtech XpressSCSI, using a Shuttle/
SCM chip set. However its firmware restricts it to only work with Jaz
drives.
On connecting the cable a message like this appears four times in the log:
reset full speed USB device number 4 using uhci_hcd
That's non-fatal but the US_FL_SINGLE_LUN quirk fixes it.

commit 823d12c95c666fa7ab7dad208d735f6bc6afabdc upstream.
People sometimes create their own custom-configured kernels and forget
to enable CONFIG_SCSI_MULTI_LUN. This causes problems when they plug
in a USB storage device (such as a card reader) with more than one
LUN.
Fortunately, we can tell fairly easily when a storage device claims to
have more than one LUN. When that happens, this patch asks the SCSI
layer to probe all the LUNs automatically, regardless of the config
setting.
The patch also updates the Kconfig help text for usb-storage,
explaining that CONFIG_SCSI_MULTI_LUN may be necessary.

commit a9c143c82608bee2a36410caa56d82cd86bdc7fa upstream.
The Cypress ATACB unusual-devs entry for the Super Top SATA bridge
causes problems. Although it was originally reported only for
bcdDevice = 0x160, its range was much larger. This resulted in a bug
report for bcdDevice 0x220, so the range was capped at 0x219. Now
Milan reports errors with bcdDevice 0x150.
Therefore this patch restricts the range to just 0x160.

commit 671b4b2ba9266cbcfe7210a704e9ea487dcaa988 upstream.
Many cards based on CY7C68300A/B/C use the USB ID 04b4:6830 but only the
B and C variants (EZ-USB AT2LP) support the ATA Command Block
functionality, according to the data sheets. The A variant (EZ-USB AT2)
locks up if ATACB is attempted, until a typical 30 seconds timeout runs
out and a USB reset is performed.
https://bugs.launchpad.net/bugs/428469
It seems that one way to spot a CY7C68300A (at least where the card
manufacturer left Cypress' EEPROM default vaules, against Cypress'
recommendations) is to look at the USB string descriptor indices.
A http://media.digikey.com/pdf/Data%20Sheets/Cypress%20PDFs/CY7C68300A.pdf
B http://www.farnell.com/datasheets/43456.pdf
C http://www.cypress.com/?rID=14189
Note that a CY7C68300B/C chip appears as CY7C68300A if it is running
in Backward Compatibility Mode, and if ATACB would be supported in this
case there is anyway no way to tell which chip it really is.
For 5 years my external USB drive has been locking up for half a minute
when plugged in and ata_id is run by udev, or anytime hdparm or similar
is run on it.
Finally looking at the /correct/ datasheet I think I found the reason. I
am aware the quirk in this patch is a bit hacky, but the hardware
manufacturers haven't made it easy for us.

commit ab4b71644a26d1ab92b987b2fd30e17c25e89f85 upstream.
This reverts commit 200e0d99 ("USB: storage: optimize to match the
Huawei USB storage devices and support new switch command" and the
followup bugfix commit cd060956 ("USB: storage: properly handle
the endian issues of idProduct").
The commit effectively added a large number of Huawei devices to
the deprecated usb-storage mode switching logic. Many of these
devices have been in use and supported by the userspace
usb_modeswitch utility for years. Forcing the switching inside
the kernel causes a number of regressions as a result of ignoring
existing onfigurations, and also completely takes away the ability
to configure mode switching per device/system/user.
Known regressions caused by this:
- Some of the devices support multiple modes, using different
switching commands. There are existing configurations taking
advantage of this.
- There is a real use case for disabling mode switching and
instead mounting the exposed storage device. This becomes
impossible with switching logic inside the usb-storage driver.
- At least on device fail as a result of the usb-storage switching
command, becoming completely unswitchable. This is possibly a
firmware bug, but still a regression because the device work as
expected using usb_modeswitch defaults.
In-kernel mode switching was deprecated years ago with the
development of the more user friendly userspace alternatives. The
existing list of devices in usb-storage was only kept to prevent
breaking already working systems. The long term plan is to remove
the list, not to add to it. Ref:

commit 4898e07174b79013afd2b422ef6c4336ef8e6783 upstream.
__do_config_autodelink passes the data variable to the transport function.
If the calling functions pass a stack variable, this will eventually trigger
a DMA-API debug backtrace for mapping stack memory in the DMA buffer. Fix
this by calling kmemdup for the passed data instead.

commit bb94a406682770a35305daaa241ccdb7cab399de upstream.
This patch (as1521b) fixes the interaction between usb-storage's
scanning thread and the freezer. The current implementation has a
race: If the device is unplugged shortly after being plugged in and
just as a system sleep begins, the scanning thread may get frozen
before the khubd task. Khubd won't be able to freeze until the
disconnect processing is complete, and the disconnect processing can't
proceed until the scanning thread finishes, so the sleep transition
will fail.
The implementation in the 3.2 kernel suffers from an additional
problem. There the scanning thread calls set_freezable_with_signal(),
and the signals sent by the freezer will mess up the thread's I/O
delays, which are all interruptible.
The solution to both problems is the same: Replace the kernel thread
used for scanning with a delayed-work routine on the system freezable
work queue. Freezable work queues have the nice property that you can
cancel a work item even while the work queue is frozen, and no signals
are needed.
The 3.2 version of this patch solves the problem in Bugzilla #42730.

The 8020i protocol (also 8070i and QIC-157) uses 12-byte commands;
shorter commands must be padded. Simon Detheridge reports that his
3-TB USB disk drive claims to use the 8020i protocol (which is
normally meant for ATAPI devices like CD drives), and because of its
large size, the disk drive requires the use of 16-byte commands.
However the usb_stor_pad12_command() routine in usb-storage always
sets the command length to 12, making the drive impossible to use.
Since the SFF-8020i specification allows for 16-byte commands in
future extensions, we may as well accept them. This patch (as1490)
changes usb_stor_pad12_command() to leave commands larger than 12
bytes alone rather than truncating them.

With module.h being implicitly everywhere via device.h, the absence
of explicitly including something for EXPORT_SYMBOL went unnoticed.
Since we are heading to fix things up and clean module.h from the
device.h file, we need to explicitly include these files now.
Use the lightweight version of the header that has just THIS_MODULE
and EXPORT_SYMBOL variants.

Scanning cannot be run during suspend or hibernation, but if
usb-stor-scan freezes another thread waiting on scanning to
complete may fail to freeze.
However, if usb-stor-scan is left freezable without ever actually
freezing then the freezer will wait on it to exit, and threads
waiting for scanning to finish will no longer be blocked. One
problem with this approach is that usb-stor-scan has a delay to
wait for devices to settle (which is currently the only point where
it can freeze). To work around this we can request that the freezer
send a fake signal when freezing, then use interruptible sleep to
wake the thread early when freezing happens.
To make this happen, the following changes are made to
usb-stor-scan:
* Use set_freezable_with_signal() instead of set_freezable() to
request a fake signal when freezing
* Use wait_event_interruptible_timeout() instead of
wait_event_freezable_timeout() to avoid freezing

Connecting the V2M to a Linux host results in a constant stream of
errors spammed to the console, all of the form
sd 1:0:0:0: ioctl_internal_command return code = 8070000
: Sense Key : 0x4 [current]
: ASC=0x0 ASCQ=0x0
The errors appear to be otherwise harmless. Add an unusual_devs entry
which eliminates all of the error messages.

It was pointed out by 'make versioncheck' that linux/version.h was not
always being included where needed and sometimes included needlessly
in drivers/usb/.
This patch fixes up the includes.
For the UVC gadget driver bits, this was ACK'ed by Laurent Pinchart.
