Send node transition events for touch events
This CL adds firing of pointerover/enter when touch
starts on an element, and pointerout/leave when touch
ends.
This CL also includes some refactoring around
our PointerEvents code. We isolated:
- PE handling details off from EventHandler,
- PE states & dispatches into PointerEventManager, and
- PE creation & id-management into PointerEventFactory.
