Add integration tests and [dis]associate typed url nodes appropriately
This fixes a problem where we were not properly updating the model associators
internal node<->id maps after syncer-initiated changes. Added DCHECKs to catch
any missing instances of this in the future.
Also made the code more robustly handle history nodes that have no visits, as I
was encountering this case locally.

TEST=Run new TypedUrl integration tests, or sync deletions from one client to another, then re-add that URL from the second client.
